Clearance Procedures & Forms

All students wishing to participate in athletics at Wheelock College must first:

• Contact coach/athletic director so that they can be listed on a roster
• Have turned in up-to-date, complete & signed Health Entrance Paperwork to student life which must include:

1. Signature from their PCP which states that they "...may participate in sports without restriction"
2. Up to date immunization records according to current Massachusetts State law
3. Additional documentation re: past injuries, surgeries and/or current medical conditions if appropriate

• Please note that a front and back copy of your health insurance is needed to participate in Wheelock Athletics.
• Complete Athletic Training Clearance Forms (see below)

• 2011-12 Rookie and Veteran Medical Forms

If you visit a physician at any point during the season, please fill out the physician visit form and hand it in to the athletic trainer upon your return.
